Scheduling Your Appointment
To schedule an appointment, call our dental office at 904-342-8423 and our staff will assist you in finding a day and time that is convenient for you.
Once the appointment is scheduled, you can download and complete our new patient forms to ensure an efficient appointment. Click the following link to download our new patient forms:
We recommend completing them prior to the appointment as it saves time during your first visit. If you're unable to download and complete the forms in advance, however, you can still save time by arriving early to your appointment to complete them.
We recommend brushing and flossing your teeth prior to your appointment so our dentist can properly examine your teeth.
When you arrive at our dental office, please inform the receptionist. Once all your paperwork is completed, you'll be invited back so your initial appointment with our dentist can commence.
It’s important that new patients understand our dentist's goal is to improve oral health and that witholding important information (such as smoking or not brushing teeth regularly) can have an impact on the treatment and services that our dentist recommends. Therefore, it’s crucial that your complete and accurate medical and dental history is disclosed on your first visit.
X-rays are taken so our dentist can properly examine your teeth, jawbone and overall bone health. If you are pregnant please notify our dentist so appropriate precautions can be taken.
On your first visit, our dentist will perform a comprehensive dental exam. During this exam, our dentist will check the mouth and teeth for any chronic or potential issues like periodontal disease (also known as gum disease) and tooth decay. Our dentist will also review the x-rays and let you know if there are any concerns. After the comprehensive exam, our dentist will formulate a custom-made treatment plan and inform you of any other recommendations.
In most cases, your first appointment will also include a thorough teeth cleaning. Dental cleanings remove plaque and tartar buildup so you can maintain a healthy smile. During this cleaning session, our dentist will remove accumulated plaque from the teeth using our dental instruments. After a thorough cleaning, our dentist will polish the teeth using a gritty toothpaste in a flavor of your choosing.
Once your x-rays, exam and cleaning are complete, our dentist may recommend further treatments or certain products to further improve your oral health.
This is the best time to ask our dentist any questions you may have about your oral health, certain procedures or even how to floss properly. Our dentist doesn’t mind demonstrating the right brushing and flossing techniques, so asking for assistance is highly encouraged.
Your Next Visit
At the conclusion of your first appointment, you'll see our receptionist to schedule your next appointment and to make any necessary payments. We will reschedule you based on our dentist’s recommendation and your availability.
